Headed2(H2) provides integrated transition case management services to meet serve student needs for post-secondary life. The below data 
elements are collected through student and or administrator activity, are updated annually and formally documented in a user friendly administrative report format. The collected data elements are:
- Post Secondary education, training, employment and independent goals
- Age appropriate transition assessments
- Transition services to include age appropriate assessment, career exploration, post-secondary exploration and planning, career skill development
- Course of Study in support of Post Secondary Goals
- Student and Agency invitation documentation to IEP
